Limitations
The primary limitation of the APRI score is its large indeterminate range. Results can also be influenced by conditions that affect AST or platelet levels independently of liver fibrosis, such as muscle injury or hematological disorders.

1. What is the difference between APRI and FIB-4?
Both are noninvasive liver fibrosis scores. APRI uses AST and platelets, while FIB-4 uses age, AST, ALT, and platelets. Generally, the FIB-4 Index is considered more accurate and is preferred for patients with nonalcoholic fatty liver disease (NAFLD/MASLD).
2. What does “AST Upper Limit of Normal (ULN)” mean?
This is the highest AST value considered “normal” by the specific laboratory that performed the test. It can vary slightly between labs, but a value of around 40 IU/L is commonly used.
3. Why is the platelet count important in liver disease?
As liver disease progresses to cirrhosis, it can cause high pressure in the portal vein (portal hypertension). This leads to an enlarged spleen (splenomegaly), which traps and removes platelets from circulation, resulting in a lower platelet count (thrombocytopenia).
4. My APRI score is 1.2. What does this mean?
A score of 1.2 falls into the “indeterminate” range (between 0.5 and 1.5). This means the APRI score alone cannot reliably predict or rule out significant fibrosis. Your doctor may recommend further testing, such as a FibroScan® (transient elastography) or another noninvasive score like FIB-4.
5. Can I use this calculator if I don’t have Hepatitis C?
